All selected participants are required to attend.

How to Apply

Organizations that wish to participate in the NIATx Opioid Treatment Provider Process Improvement Learning Collaborative, Cohort II must:

  2. Identify key staff who will participate in the project. Click here for instructions.
  3. Complete the application form.
  4. Submit applications by close of business on April 29, 2011, via e-mail to: jeanne.pulvermacher@chess.wisc.edu
